CHS Healthcare is currently working with 63 different hospitals across the UK to reduce delayed discharges and avoid unnecessary admissions.
Every month, we receive between 180 and 200 referrals for hospital patients who need support to find care within the community. This may involve helping the patient’s family to identify the best care home to meet their needs, or arranging domiciliary care.
We also deliver admission avoidance schemes, arranging care within the community for those who need more support and would otherwise come into hospital.
A very common cause of a delay in discharge is waiting for a care home placement to be chosen or to become available. We provide the support for families and expertise to find the right care and speed up the process.
We have extensive experience of delivering interim care schemes for NHS hospitals across the UK. Interim care, sometimes known as step down care, involves identifying appropriate beds for patients who no longer need acute hospital care, but are not able to return to their homes.
We have extensive experience of arranging respite care and can organise care at short notice because we monitor bed availability and are in contact with homes on a weekly basis.
