Help with care in your own home

We have extensive experience of arranging care for people within their own homes, enabling them to continue to live independently with support. We can liaise with care agencies to organise packages and monitor the quality of care, dealing with any problems if they arise.

How does our service work?

Many people with personal and nursing care needs want to return to their own home after a period in hospital, or continue to live in their own home. In order to do this, a package of care needs to be arranged to offer extra support to meet their needs. This may involve carers coming into the home on a daily basis, or several days a week, to help with day-to-day tasks. Some people will require help from nurses, who may be needed to help with washing, dressing and nursing care.

CHS has been helping people to arrange care in their own homes, sometimes called domiciliary care, for many years. We have links with primary care trusts, social services and many care agencies and help you through what can seem a difficult and confusing process or arranging care. We know the agencies which specialise in more complex nursing care and we can ensure that your personal preferences and wishes are respected.

Importantly, we continue to monitor the care that is provided. We will call you every month to see how you are getting on and you can call us as often as you like. We can quickly sort out any problems which arise and if needs change, such as you or your relative require more support, we can liaise with all the agencies involved and organise a new care package.
