Carehome services for Social Services
CHS Healthcare offers a range of programmes to support social services in the delivery of social care.
We have a long track record of working closely with social services in hospital and within the community. We understand continuing care and have been working with care providers since 1995. We also have extensive experience of partnership working with the NHS and local authorities.
Self-directed support
We understand self-directed support and have already been commissioned by social services and the NHS to work in this field. We can carry out both key roles – brokering care and financial management.
Hospital discharge co-ordinator
We are specialists in organising and supporting the process of discharge from hospital. We have been commissioned by social services to co-ordinate this process for patients moving to care homes and those who need domiciliary care.
